An exciting opportunity has arisen for an Occupational Therapist to join our dynamic and fast-paced Integrated Discharge Service in Islington. Join our talented team which consists of about 10 therapists, including a Senior Physiotherapist and a Senior Social Worker, who are committed to supporting residents making the transition from hospital to home, seven days a week. Being a part of this team, you will play a vital role in assessing risks and designing interventions for residents at various stages of the hospital pathway. By leveraging residents' strengths, you will help them in achieving their outcomes.
What you will do:
- Support the Discharge to Assess (D2A) pathway through effective screening of referral documentation.
- Make informed decisions on planning and supporting discharge processes.
- Collaborate with multidisciplinary teams to provide holistic patient care.
- Implement therapeutic interventions tailored to individual patient needs.
- Monitor and review patient progress, adjusting care plans as necessary.
Requirements of the Occupational Therapist:
- A degree or equivalent in Occupational Therapy.
- Recent UK experience working as an Occupational Therapist.
- Recent NHS experience is desirable.
- Registered with the Health and Care Professions Council (HCPC).
