What you will do:

• Conduct comprehensive mental health assessments to devise effective treatment plans for adults within the community.
• Manage a caseload as a Care Coordinator and key worker, offering tailored support to suit individual client needs.
• Undertake duty work and responsibly handle the screening of new referrals to the team.
• Administer depot injections and manage Community Treatment Order cases efficiently.
• Participate in ward rounds and multidisciplinary team (MDT) discussions to ensure cohesive and informed patient care.
