What you will do:

  • Manage a caseload of young people aged 16 to 17, providing tailored support and leadership in planning their pathways.
  • Guide these individuals through crucial transitions, such as moving from residential care to supported accommodation and navigating their journey from 17 to 18.
  • Collaborate proactively with residential homes, supported accommodation providers, and partner agencies, ensuring seamless transitions and coordinated plans.
  • Co-produce safety plans with young people to promote confidence, independence, and personal safety.
  • Foster strong, reliable relationships, offering stability, belonging, and emotional security to the young people you support.
  • Engage in reflective practice sessions, development days, and group supervisions to continually evolve and enhance practice.
  • Utilise quality assurance tools to guarantee timely and effective planning for strong outcomes.
  • Actively promote participation, ensuring the young person's voice influences service development and the advancement of the Super Hub.
