What you will do:

Conduct statutory reviews for citizens residing in care home placements.
Collaborate with health partners to facilitate status changes for care homes, including Continuing Health Care.
Serve as a contact point for care home managers, providing support to social work colleagues during unplanned reviews.
Support provider performance, including managing processes for care home closures to aid citizens and families effectively.
Maintain a consistent approach towards providers, working in tandem with Contracts and Commissioning colleagues to uphold best practices.
Actively engage in benchmarking and knowledge development in dementia care, promoting best practices across the service.
