What you will do:
- Deliver robust, high-quality Care Act (2014) assessments, carer assessments, and support planning aimed at fostering independence and achieving personal outcomes for service users.
- Skillfully manage a complex caseload involving adult social care, high-risk safeguarding, and transitions from children’s services.
- Collaborate effectively with multi-agency teams, including health services, housing, and care providers, to ensure comprehensive care solutions.
- Conduct comprehensive mental capacity assessments, manage Deprivation of Liberty Safeguards, and lead Best Interests decision-making processes.
- Implement dynamic safety plans in collaboration with multi-agency partners, engaging in crisis management and escalation when necessary.
- Contribute to multi-agency panels and funding assessments to secure care funding and develop effective service packages.
- Engage regularly with staff at all levels in Adult Social Care, Head of Service, Health Authority partners, NHS Trust staff, care providers, legal services, police, emergency teams, service users, and family carers to foster meaningful collaborative relationships.
