What you will do:

  • Lead a person-centred service for adults with learning disabilities, ensuring that all duties align with the Care Act 2014 guidelines and council policies.
  • Undertake complex cases, providing a high level of professional service and championing best practices in social work.
  • Facilitate service user choice and independence, promoting the personalisation agenda at all stages of assessment.
  • Supervise conditionally discharged patients if required, applying your expertise in social supervision.
  • Undertake statutory interventions where service users are at risk, ensuring adherence to safeguarding policies and procedures.
  • Ensure information is recorded accurately and consistently, maintaining confidentiality and sharing relevant information with carers and agencies per data protection requirements.
