What you will do
- Manage a complex caseload of children and young people with disabilities and additional needs.
- Undertake comprehensive assessments of children and families, identifying strengths, needs, risks, and desired outcomes.
- Develop, implement, and review child-centred care and support plans.
- Ensure the wishes, feelings, views, and lived experiences of children and young people are central to assessment and decision-making.
- Undertake statutory visits and reviews in accordance with legislation, policy, and required timescales.
- Assess safeguarding concerns and take appropriate action to protect children from abuse, neglect, exploitation, or other risks.
- Coordinate multi-agency plans involving health, education, social care, therapy, and other specialist services.
- Work collaboratively with parents, carers, schools, health professionals, and other agencies.
- Support transitions for young people between children's and adult services where appropriate.
- Prepare high-quality assessments, reports, and plans for reviews, panels, conferences, and court proceedings.
- Provide professional advice, guidance, and support to colleagues on complex cases.
- Contribute to team development, practice improvement, and service development initiatives.
Person Specification
Essential Qualifications:
- Professional Qualification: Recognised professional qualification in Social Work, such as BA/BSc/MA in Social Work.
- Registration: Current registration with Social Work England.
- Post-Qualifying Experience: Previous post-qualifying experience in children's social care.
- Legislative Knowledge: Understanding of legislation and statutory guidance for children's social care.
Essential Experience:
- Children's Social Care: Experience working with children, young people, and families in a statutory social care setting.
- Disability Services: Experience supporting children and young people with disabilities or complex needs.
- Multi-Agency Working: Experience of safeguarding children and working effectively within multidisciplinary partnerships.
