What you will do:

  • Conduct comprehensive Care Act Reviews to assess whether individuals' care and support arrangements continue to meet their eligible needs.
  • Work collaboratively with adults, families, carers, and other professionals to develop personalised care and support plans.
  • Assess changing needs and identify appropriate services, resources, and support options.
  • Review existing care packages and recommend adjustments where required.
  • Promote independence, wellbeing, choice, and control in accordance with Care Act principles.
  • Ensure assessments and reviews are completed within statutory and organisational timescales.
  • Maintain accurate and up-to-date case records, assessments, and care plans.
  • Provide professional advice and support to individuals, families, and carers.
  • Work effectively with multidisciplinary teams and external agencies to achieve positive outcomes.
  • Ensure practice complies with relevant legislation, policies, and safeguarding requirements.
