What you will do:

  • Manage a complex caseload of children and young people with disabilities and their families, ensuring their voices are heard and their needs are appropriately met.

  • Undertake statutory assessments including Child in Need (CIN), Child Protection (CP), Looked After Children (LAC), and Care Act transition assessments.

  • Develop, implement, and review child-centred care plans that promote safety, wellbeing, and independence.

  • Complete safeguarding enquiries, risk assessments, statutory visits, and court reports to ensure effective support and protection.

  • Work collaboratively with education services, health professionals, SEND teams, CAMHS, and other partner agencies to deliver coordinated support.

  • Support young people and families through transition to adult services, helping to ensure effective preparation for adulthood.

  • Provide mentoring, guidance, and support to less experienced social workers, students, and colleagues where required.

  • Maintain accurate, timely, and comprehensive case records in line with statutory guidance, council policies, and Social Work England standards.

  • Participate in strategy meetings, review meetings, child protection conferences, and multi-agency planning forums.

  • Promote strengths-based, child-centred practice that achieves positive and sustainable outcomes.
