What you will do:
- Coordinate and facilitate the assessment of needs, and develop education, health, and care plans that are outcome-focused.
- Act as a keyworker for children and adults with special educational needs and disabilities, coordinating with other professionals including social workers and educational psychologists.
- Develop collaborative relationships with individuals and their families to promote choice, independence, and respect for diversity.
- Analyze information to understand needs and strengths, identify risks, and develop strategies for risk management.
- Collaborate with various professionals to ensure comprehensive support and service delivery for individuals.
- Manage and review personal budgets and funding allocations, ensuring transparent communication regarding costs to families.
- Document all key discussions and assessments accurately in line with policy and procedures.
- Conduct annual reviews of individual plans and progress, adjusting strategies as needed to ensure positive outcomes.
- Provide interim support and monitoring to address immediate needs of individuals or families.
