This is an excellent opportunity for a qualified Adults Social Worker to join the Review Team based at County Hall, HU17 9BA. You will play a crucial role in reviewing care and support arrangements, ensuring adults receive appropriate, person‑centred services that promote independence and wellbeing.

What you will do

• Manage a varied caseload and complete statutory reviews to ensure care and support remain appropriate and effective.
• Undertake assessments and develop individualised care and support plans aligned with identified needs.
• Work collaboratively with health professionals, care providers, and partner agencies.
• Promote safeguarding by identifying risks and ensuring appropriate action is taken where required.
• Attend multi‑agency meetings and engage with community resources to support holistic service delivery.
• Maintain accurate, timely, and compliant case records in line with statutory and organisational requirements.

Requirements

• Qualified Social Worker with current Social Work England registration.
• Previous experience within Adult Social Care.
• Ability to manage reviews and assessments effectively within a statutory framework.
• Willingness to submit a CV for consideration and attend an interview.
• Photographic identification required on the first day for security compliance.
