What You Will Do

• Conduct comprehensive functional assessments and develop personalised, goal-oriented reablement plans.
• Collaborate with reablement carers, physiotherapists, and other multidisciplinary professionals to deliver holistic care.
• Provide clinical coaching on rehabilitative techniques to support client progress.
• Recommend, fit, and guide clients on low-level equipment such as mobility aids and minor adaptations for accessibility.
• Manage your caseload independently, including travelling to peripheral locations to collect and deliver equipment.

Additional Requirements

• HCPC registration is mandatory.
• Car driver essential due to travel requirements.
