About the Role

This full-time role provides a remarkable opportunity to work with the East Community Hospital and Transfer of Care team, supporting adults across both inpatient and community settings. What You Will Do

Deliver statutory social work by managing a varied and complex caseload.
Conduct Care Act assessments, care planning, and reviews.
Work closely with multi-disciplinary teams, including health and community partners, supporting individuals through rehabilitation and reablement.
Play a key role in safeguarding and protecting vulnerable adults.
Provide guidance, mentorship, and support to colleagues and junior practitioners.
Exercise strong professional judgement in a person-centred, anti-oppressive practice.

Requirements

Qualified Social Worker (e.g., DipSW, CQSW, BA Social Work).
Registered with Social Work England.
Demonstrable experience in Adult Social Care, particularly with complex cases.
Thorough understanding of relevant legislation, including the Care Act and Mental Capacity Act.
Ability to work independently and as part of a team.
