What you will do:
- Assess and provide tailored physiotherapy interventions to individuals within the reablement service.
- Work collaboratively with an MDT (multi-disciplinary team) to develop integrated care plans.
- Support patients in community settings, assisting in their rehabilitation and reablement.
- Document patient progress and offer insights for case discussions.
- Provide guidance and advice to patients and their families, enhancing their rehabilitation journey.

Requirements of the Physiotherapist:
- Must possess a valid UK driving licence as a car driver.
- Registration with the Health and Care Professions Council (HCPC) is essential.
- Experience working within a multi-disciplinary team and in rehabilitation is preferable.
- Seeking a candidate with some post-graduate experience to manage complex team caseloads effectively.
- Ability to work across all pathways of Intermediate Care, with some community experience required.
