Collaborating closely with local hospitals, Bolton at Home, and other key partners, you will play a pivotal role across DTAH and the wider independent living services. What you will do:

• Work alongside the independent living service team to provide comprehensive physiotherapy assessments and interventions.
• Facilitate smooth and effective patient discharges through the Discharge to Assess at Home (DTAH) protocol.
• Deliver exceptional telecare and sensory services as part of a holistic approach to community social services.
• Collaborate with hospitals, Bolton at Home, and other partners to enhance patient outcomes.

• Maintain accurate and up-to-date records in line with service protocols and HCPC guidelines.
