What you will do:

• Undertake Care Act assessments, reviews and support planning for adults within acute hospital settings.
• Facilitate safe and timely hospital discharges in partnership with healthcare professionals, community providers, and family members.
• Complete mental capacity assessments and best-interest decision-making processes where appropriate.
• Manage a complex caseload involving older adults, individuals with physical disabilities, mental health needs, and long-term conditions.
• Participate in multidisciplinary team (MDT) meetings, discharge planning forums, and safeguarding discussions.

Roles and Responsibilities:

• Undertake Care Act assessments, reviews, and support planning for adults within acute hospital settings.
• Facilitate safe and timely hospital discharges in partnership with healthcare professionals, community providers, and family members.

Person Specification:

• Degree in Social Work, DipSW, CQSW, or equivalent recognised Social Work qualification.
• Evidence of continuing professional development relevant to adult social care and hospital discharge services.
• Current registration with Social Work England.
• Post-qualification experience within Adult Social Care, Hospital Discharge, Community Teams, or Integrated Care Services.
• Experience undertaking Care Act assessments and managing complex adult social care cases.
