What you will do:
- Implement reablement plans developed by the Occupational Therapist to support clients in regaining their independence.
- Ensure that any specialist equipment provided meets the intended purpose of enhancing independence and ease of care safely and effectively.
- Provide accurate and objective verbal and written reports, maintaining IT records and correspondence to agreed standards and time-frames.
- Adhere to the council’s confidentiality and data protection guidelines.
- Make sound decisions through the use of clinical reasoning.
- Maintain robust and clear recording on the database.
- Collaborate effectively with a wide range of stakeholders at all levels across various organisations.
Qualifications and Skills:
- UK/NHS experience would be an advantage but is not essential.
- Professional HCPC Registration is essential to practice in this role.
