What you will do:
- As a Specialist Dementia Nurse, you will monitor and coordinate the care of clients on anti-dementia medication and manage a caseload referred to memory services. You will:
- Conduct thorough assessments and interventions and provide follow-up and support for patients.
- Collaborate with a multidisciplinary team and liaise with external agencies to maintain high standards of care.
- Serve as a care coordinator, ensuring all client needs are planned and reviewed using CPA guidelines.
- Administer medications in line with Trust policies, and offer education and support to clients and carers.
- Act as a resource by providing information, training, and health promotion for clients and healthcare professionals.
- Enhance documentation on care pathways and adhere to Trust-approved procedures and protocols.
Your communication skills will be vital as you build positive relationships with clients, carers, and a diverse professional team. You'll also contribute to service development, share specialist knowledge, and stay informed about advancements in dementia care.
