This is an exciting opportunity for an experienced Social Worker to join the Home First Hospital Team in the stunning region of East Lothian. Based across John Muir House, East Lothian Community Hospital, and occasionally the Royal Infirmary of Edinburgh, this role sits within a fast-paced Unscheduled Care service, embracing the Home First approach to support safe, timely discharges and prevent unnecessary hospital admissions.

You’ll be working in a dynamic, multidisciplinary environment where your professional judgement, adaptability, and commitment to person-centred care will be highly valued.

What you will do:

• Respond promptly to referrals via daily multidisciplinary team meetings

• Deliver short-term, focused interventions to stabilise situations and support safe outcomes

• Work collaboratively with MDT colleagues to prevent unnecessary acute hospital admissions

• Support A&E turnaround, assisting patients to avoid ward admission where appropriate

• Apply strong decision-making skills in a fast-moving, unscheduled care setting

• Travel between sites across East Lothian, using your car and driving licence to provide timely support

• Utilise MOSAIC experience where applicable (desirable but not essential)
