What you will do:

- Facilitate Pathway 1 hospital discharges by engaging with customers on the day of discharge.
- Conduct comprehensive assessments and develop support plans to aid clients’ reablement.
- Collaborate closely with multidisciplinary teams to ensure holistic care approaches.
- Monitor and evaluate clients' progress, adjusting plans as required to ensure optimal outcomes.
- Provide guidance and educational support to clients and their families to promote health and wellbeing.
