What you will do:

  • Manage and prioritise a caseload requiring assessment, review, and on-going support to achieve improved outcomes for adults over the age of 18
  • Conduct home visits weekly in person to provide direct support to service users
  • Prescribe, order, and recommend specialist disability equipment, minor adaptations, and standard wheelchairs. Assist adults and their carers in safe installation, maintenance, and use of equipment
  • Review and update the Mosaic system with accurate case notes and documentation
  • Conduct Moving and Handling Risk Assessments, provide recommendations, training, and develop Personal Handling Plans to support both adults and carers

Requirements:

  • Hold a relevant professional qualification in Occupational Therapy and current HCPC registration
  • Demonstrated experience with reablement and/or within a Local Authority setting is preferred
  • Possess an in-depth knowledge of relevant social care and health legislation
  • Ability to produce high-quality clinical reports and manage a substantial amount of administrative work efficiently
  • Car driver preferred as home visits will be an integral part of the role
