What you will do:

• Assess and develop treatment plans to promote patient recovery within the Intermediate Care Team

• Collaborate with multidisciplinary teams to tailor interventions and enhance patient outcomes

• Provide advice and support to patients, carers, and families to encourage progress within care plans

• Monitor and review patient progress and adapt treatment accordingly

• Document patient progress and treatment effectiveness, ensuring accurate and detailed records are maintained

• Engage in team meetings and professional development sessions to support continuous service improvement
