What you will do:

Conduct home assessments to determine individuals’ needs and recommend suitable adaptations or equipment

Collaborate with clients, carers, and other healthcare professionals to create personalised intervention plans that maximise independence

Oversee the implementation of home adaptations, ensuring compliance with relevant regulations and client requirements

Provide expert guidance and support to Social Services in complex cases

Maintain accurate and detailed case records, documenting interventions and outcomes

Deliver training to Social Services staff to enhance understanding of occupational therapy within the home adaptations setting
