What you will do:

• Conduct home assessments to determine individuals’ needs and recommend suitable adaptations or equipment

• Collaborate with clients, carers, and other healthcare professionals to create personalised intervention plans that maximise independence

• Oversee the implementation of home adaptations, ensuring compliance with relevant regulations and client requirements

• Provide expert guidance and support to Social Services in complex cases

• Maintain accurate and detailed case records, documenting interventions and outcomes

• Deliver training to Social Services staff to enhance understanding of occupational therapy within the home adaptations setting
