What you will do
• Caseload Management: Manage a caseload of patients recovering from stroke and neurological conditions, acting as a key worker where appropriate.
• Occupational Therapy Assessment: Complete comprehensive assessments to identify patient needs, strengths, and rehabilitation goals.
• Treatment Planning: Develop person-centred goals and treatment plans in partnership with patients and their carers.
• Intervention Delivery: Implement evidence-based occupational therapy interventions to maximise independence and functional outcomes.
• Outcome Monitoring: Evaluate progress and adapt treatment plans to meet changing patient needs.
• Multidisciplinary Working: Work collaboratively with physiotherapists, nurses, speech and language therapists, and other healthcare professionals.
• Discharge Planning: Lead and support discharge planning processes to ensure safe and effective transitions from inpatient services.
• Patient Advocacy: Promote patient participation, independence, and engagement in meaningful daily activities and life roles.
• Documentation: Maintain accurate clinical records and service documentation in line with Trust requirements.
• Compliance: Ensure practice aligns with national guidelines, legislation, professional standards, and organisational policies.
Qualifications / Requirements
• Occupational Therapy Qualification: Degree in Occupational Therapy or equivalent recognised qualification.
• Registration: Current HCPC registration as an Occupational Therapist.
• Neuro-Rehabilitation Experience: Previous experience working with neurological or stroke rehabilitation caseloads.
• Clinical Knowledge: Strong understanding of neuro-anatomy, stroke presentations, and rehabilitation principles.
• Moving and Handling Skills: Experience applying moving and handling techniques within clinical and community settings.
• Communication Skills: Excellent interpersonal, assessment, and report-writing abilities.
• Driving Licence: Full UK driving licence and access to a vehicle are essential.
• Multidisciplinary Working: Ability to work effectively within integrated healthcare teams.
