What You Will Do

• Assess, evaluate, and review service users’ needs in the community.

• Develop and implement individualised, person-centred treatment plans to promote independence and safety.

• Undertake community visits (access to a vehicle with business insurance is required).

• Work closely with the multidisciplinary team to ensure coordinated and effective care delivery.

• Provide education, advice, and support to service users and their families/carers.

• Maintain accurate and timely clinical documentation in line with professional standards.

• Keep up to date with best practice and evidence-based occupational therapy interventions.
