What you will do
- Complete Adult Social Care Occupational Therapy assessments to ensure personalised care plans.
- Undertake DHC to SHC reviews and optimise packages of care for individuals in need.
- Carry out complex moving and handling assessments with a focus on safety and effectiveness.
- Collaborate closely with home care providers, care coordinators, individuals, and families.
- Assess for and recommend the use of specialist equipment to enhance daily living.
- Conduct postural and seating assessments for improved comfort and functionality.
- Design and produce basic drawings for standard adaptations including LAS.
- Evaluate and present recommendations for major adaptations where necessary.
- Manage an independent caseload while meeting service deadlines.
- Prepare comprehensive reports, maintain accurate records, and handle correspondence professionally.
- Liaise with service users, carers, and multidisciplinary professionals for cohesive care delivery.
- Support Occupational Therapy students and support workers, providing guidance when appropriate.
