What you will do

  • Undertake comprehensive Occupational Therapy assessments for individuals receiving bridging support to ensure timely and effective care planning.
  • Facilitate progression into Reablement services, continuously identifying opportunities to maximise independence and reduce ongoing care needs.
  • Assess and recommend necessary equipment and adaptations, ensuring individuals have the resources required for their rehabilitation journey.
  • Perform moving and handling assessments, delivering rehabilitation-focused interventions that promote safety and independence.
  • Collaborate closely with Reablement staff and partner agencies, contributing to risk assessments and the maintenance of accurate clinical records and documentation.

Qualifications / Requirements

  • A degree or diploma in Occupational Therapy and HCPC registration.
  • Experience in health or social care settings, including Occupational Therapy assessments and recommending equipment and adaptations.
  • Experience in undertaking moving and handling assessments and driving independence-focused interventions.
  • Strong communication and record-keeping skills are essential, along with a full UK driving licence and access to a vehicle.
  • Desirable: Experience in Reablement, community Occupational Therapy, adult social care, and working within multidisciplinary teams.
