What you will do:
- Assess adults and children with physical and learning disabilities using a strengths-based approach.
- Develop, implement, review, and monitor personalised care plans.
- Assess for and recommend equipment, adaptations, and moving and handling solutions to enhance independence.
- Commission services related to equipment and adaptations tailored to individual needs.
- Conduct moving and handling assessments and devise appropriate plans.
- Provide expertise as Duty Officer, screening referrals and offering specialist advice.
- Liaise with statutory and voluntary agencies to ensure comprehensive care.
- Participate actively in reviews, strategy meetings, and case conferences to safeguard vulnerable individuals.
- Provide professional guidance and consultation to care managers and colleagues.
- Supervise Occupational Therapy students to foster a culture of learning.
- Maintain accurate, detailed records and case documentation for transparency and accountability.
Requirements:
- A recognised Occupational Therapy qualification and HCPC registration.
- Knowledge of relevant legislation including the Children Act 1989, Care Act 2014, and Mental Capacity Act.
- Experience with safeguarding practices and the ability to undertake comprehensive assessments.
- Strong verbal and written communication skills, coupled with excellent team collaboration.
- Capability to travel across various locations within Harrow.
