What you will do:

  • Provide advice, guidance, and practical support to care leavers in areas including accommodation, financial management, education, training, and employment.

  • Support young people in developing independence skills and achieving their personal goals.

  • Contribute to the assessment, development, implementation, and review of Pathway Plans, ensuring they remain person-centred and outcome-focused.

  • Maintain regular contact with care leavers, providing ongoing encouragement and support throughout their transition to adulthood.

  • Facilitate and supervise contact between care leavers and their birth families where appropriate.

  • Encourage and coordinate access to support services, community resources, and specialist interventions.

  • Promote positive physical, emotional, and mental wellbeing, signposting young people to relevant health services when required.

  • Empower young people to make informed decisions, build resilience, and take greater control of their futures.

  • Identify safeguarding concerns and manage risks in accordance with legislation, policy, and local procedures.

  • Work collaboratively with colleagues, partner agencies, community organisations, education providers, and local support networks.

  • Maintain accurate, secure, and up-to-date records in line with organisational requirements and data protection legislation.

  • Participate in team meetings, training, and service development initiatives to support continuous improvement.
