Occupational Therapist within the Equipment & Adaptations Team in Crewe, earning £35.00 per hour. What you will do:

Assess the needs of adults with illnesses, disabilities, or long-term conditions, identifying appropriate equipment and adaptation solutions.

Provide expert information, guidance, and advice to service users, families, and carers.

Undertake assessments for individuals returning home from hospital or residential settings, ensuring suitable equipment and support are in place.

Recommend, prescribe, and review specialist equipment to maximise safety and independence.

Work closely with Housing Services, NHS teams, contractors, and other partner organisations to coordinate adaptation projects.

Liaise with architects, surveyors, service users, and other stakeholders to review and approve adaptation plans.

Monitor completed adaptations to ensure they effectively meet assessed needs and deliver positive outcomes.

Keep up to date with developments in equipment, adaptations, and Occupational Therapy practice, organising demonstrations and training where appropriate.

Identify rehabilitation potential and make referrals to relevant rehabilitation and support services.

Maintain accurate, timely, and comprehensive case records, assessments, and reports in accordance with professional and organisational standards.

Contribute to multidisciplinary team discussions and promote person-centred, strengths-based practice.
Requirements:

Degree or equivalent qualification in Occupational Therapy.

Current HCPC registration.

Experience within community Occupational Therapy, equipment provision, adaptations, or adult social care services.

Strong assessment, problem-solving, and clinical reasoning skills.

Experience working collaboratively with housing services and multidisciplinary professionals.

Excellent communication, organisational, and report-writing abilities.

Full UK driving licence and access to a vehicle may be required.
