What you will do:

  • Conduct comprehensive assessments and triage of adults' needs, risks, and support requirements.

  • Analyse information from adults, carers, families, and partner agencies to inform decision-making and intervention planning.

  • Develop and contribute to effective care and support plans that promote independence, wellbeing, and positive outcomes.

  • Work collaboratively with multidisciplinary teams and partner organisations to coordinate services and interventions.

  • Advocate for adults and carers, ensuring their wishes, views, and outcomes remain central to all assessment and care planning processes.

  • Provide professional advice, guidance, and consultation to colleagues and external agencies.

  • Respond appropriately to safeguarding concerns and contribute to safeguarding enquiries in line with relevant legislation and procedures.

  • Manage a varied caseload efficiently, ensuring statutory duties and timescales are met.

  • Complete strengths-based assessments in accordance with the Care Act 2014 and associated legislation.

  • Maintain accurate, detailed, and timely case records, assessments, and reports.

  • Promote strengths-based and person-centred practice throughout all aspects of service delivery.
