Job Title: Band 6 Community Psychiatric Nurse - Older Adults Dementia and Diagnosis

Salary: £26 per hour, Hourly
Are you ready to embark on an exciting new opportunity as a Band 6 Community Psychiatric Nurse within the Older Adults Dementia and Diagnosis team in Telford? This role offers a rewarding chance to make a significant impact on the lives of older adults living with dementia, contributing to their diagnosis and care. The initial contract is expected to last 3 to 6 months, with a strong possibility of extension if successful and the need persists. The primary working hours are 09:00- 17:00, Monday to Friday, but flexible hours such as 08:00 – 18:00 x 4 days or 09:00 – 16:00 are also open for negotiation.

What you will do:
- Manage a complex caseload, including conducting specialist assessments, developing care plans with service users and carers, and evaluating individual treatment plans.
- Participate in setting quality standards and auditing clinical practices for continuous improvement.
- Administer medication in line with trust procedures and NMC guidelines.
- Develop, implement, and evaluate patient care programmes tailored to individual needs.
- Deliver various interventions, including pharmacological and psychologically informed methods, aligned with care plans and Pathway guidance.
- Conduct risk assessments and develop risk management plans to enhance the quality of life and occupational independence.
- Maintain effective communication and reporting, observing patient conditions and documenting progress.
- Use outcome measures to assess the effectiveness of interventions as part of the care planning process.
- Contribute to the service's development, actively participating in clinical governance activities at the team level.
- Uphold the NMC Code of Practice and maintain professional competence.
- Support the implementation of evidence-based nursing practices and participate in relevant research projects.
Requirements of the Community Psychiatric Nurse:
- A degree or equivalent qualification in Mental Health Nursing.
- NMC Registration and post-registration experience, with evidence of diploma/degree-level courses and training.
- Extensive experience working with adults experiencing complex mental health needs and delivering interventions in a community setting.
- A proven ability to work autonomously and engage with individuals who have difficulty accessing services.
