Pay Rate: PAYE £28.28 per hour | Umbrella £38.02 per hour
Seven Resourcing are currently recruiting for an experienced Social Worker to join a Leaving Care Service within Children in Care services in Swindon. This is an excellent opportunity for a qualified Social Worker with experience in Children’s Social Care, Leaving Care, Pathway Planning, and Children in Care services.
The successful Social Worker will work within the Positive Futures service, supporting young people who are transitioning from care into independent living. You will play a key role in ensuring effective pathway planning, safeguarding, and ongoing support for care experienced young people.
Job Purpose
To manage a caseload of care experienced young people preparing to leave care, ensuring high-quality pathway planning, assessments, and ongoing support in line with statutory Children in Care and Leaving Care legislation. The Social Worker will work closely with Pathway Advisors and multi-agency professionals to ensure smooth transitions into adulthood.
Key Responsibilities
- Manage an allocated caseload of young people within a Leaving Care / Children in Care service
- Complete and lead on pathway plan assessments and initial pathway plans
- Maintain and review pathway plans in line with statutory requirements and legislation
- Work collaboratively with Pathway Advisors and other Children’s Social Care professionals
- Support young people in preparation for independence and adulthood
- Deliver direct Social Work intervention and ongoing case management
- Participate in a duty system providing additional support to care experienced young people
- Ensure safeguarding responsibilities are met at all times
- Maintain accurate, timely, and compliant case recording and documentation
- Liaise with multi-agency partners including education, housing, health, and support services
Candidate Requirements
- Qualified Social Worker with Social Work England registration (or equivalent)
- Experience within Children in Care, Leaving Care, or Children’s Social Care services
- Strong understanding of Pathway Planning and care leaver legislation
- Experience managing a caseload of vulnerable young people
- Excellent assessment, communication, and report writing skills
- Ability to work collaboratively within multi-agency settings
- Strong safeguarding knowledge and practice
- Commitment to improving outcomes for care experienced young people
