What you will do:

  • Conduct thorough social care assessments, ensuring timely and accurate delivery of care packages for hospital inpatients before their discharge.
  • Perform mental capacity assessments, best interest decisions, and safeguarding measures as required to maintain the highest standards of care.
  • Attend Section 117 meetings, ward rounds, and professional gatherings to offer insights and develop holistic care plans.
  • Support the discharge processes from PICU and acute wards, facilitating smooth transitions for service users.
  • Complete comprehensive Ministry of Justice Reports to uphold legal standards and requirements.
  • Participate actively in the duty rota to ensure consistent service delivery.
  • While not mandatory, having a car and experience with Liquid Logic LAS is preferred, adding to your ability to navigate services efficiently.
