What you will do:

- Deliver therapeutic interventions for patients on the ICT pathway 1
- Equip patients with the necessary medical equipment required for their progress
- Monitor and review patient progress to ensure optimal recovery
- Support the development of Integrated Care Work (ICW) processes and procedures
- Enhance patient flow to streamline healthcare delivery
- Provide dedicated support to all patients on step-up and step-down pathways

Requirements:

- Registered with the Health and Care Professions Council (HCPC)
- Minimum of 2 years of experience in a relevant setting
- Car driver preferable to maneuver efficiently across our county-wide facilities
