About the role
An exciting opportunity has arisen for an experienced Adult Social Worker to join the Mental Health Review Team in Lancashire. This role focuses on statutory reviews and reassessments for adults with mental health needs, supporting recovery, independence, and wellbeing through a strengths-based and person-centred approach.
You will work collaboratively within a multidisciplinary setting, ensuring care and support plans remain proportionate, effective, and compliant with statutory duties, while making a meaningful difference to individuals’ lives across the county.

Role purpose
As an Adult Social Worker within the Mental Health Review Team, you will undertake statutory reviews and reassessments under the Care Act 2014, promoting recovery and independence while ensuring safeguarding responsibilities and legal frameworks are met.
What you will do

Complete statutory reviews and reassessments for adults with mental health needs under the Care Act 2014.

Carry out Mental Capacity Assessments and lead Best Interests decision-making when required.

Undertake safeguarding enquiries in line with Section 42 duties.

Review and update care and support plans to ensure they are person-centred, proportionate, and outcome-focused.

Work collaboratively with Community Mental Health Teams, health professionals, housing services, and voluntary sector partners.

Support service users using a recovery-focused, strengths-based approach.

Maintain accurate, timely case records in accordance with council and professional standards.

Provide advice, information, and guidance to service users, carers, and families.

Participate in duty systems, team meetings, and service development activities.

Maintain professional registration and actively engage in continuous professional development.
Essential requirements

Degree or equivalent qualification in Social Work.

Registered with Social Work England.

Experience working with adults within mental health services.

Strong working knowledge of the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults legislation.

Experience completing reviews, reassessments, and care planning.

Ability to work effectively within a multidisciplinary environment.

Excellent written and verbal communication skills.
Desirable

Experience within a Mental Health Review Team or CMHT setting.

Knowledge of Mental Health Act pathways and processes.

Experience managing complex or high-risk cases.

Familiarity with local authority case management systems.
