Job Overview

An exciting opportunity has arisen for a Band 6 Physiotherapist to join the Discharge Planning Team in Epsom.

This ongoing, full-time locum role offers a competitive hourly rate of £29.74 and the chance to work in a fast-paced acute environment where you will play a key role in supporting safe, timely, and effective hospital discharges.

You will work across medical wards, helping patients regain independence and ensuring smooth transitions from hospital to home or onward care.

What You Will Do

Manage discharge planning for a varied caseload of patients, ensuring safe and timely discharges.
Provide specialist physiotherapy assessment and treatment to patients across medical wards.
Work collaboratively within a multidisciplinary team to develop and implement effective discharge and rehabilitation plans.
Complete functional assessments and implement treatment programmes to maximise patient independence.
Maintain accurate and timely clinical documentation in line with professional and organisational standards.
Contribute to service efficiency and patient flow across the hospital.

Requirements

Essential

HCPC registration.
Full UK driving licence (required to travel between locations).
Previous experience working at Band 6 level in an acute or discharge-focused setting is desirable.
