What you will do

• Assessment and Rehabilitation: Undertake comprehensive occupational therapy assessments and deliver rehabilitation interventions for patients with complex needs.
• Hospital Discharge Planning: Lead and contribute to effective discharge planning, ensuring safe transitions from hospital to home or alternative settings.
• Caseload Management: Manage a specialist caseload within acute and physical health environments, prioritising workloads effectively.
• Clinical Decision-Making: Apply advanced clinical reasoning skills and evidence-based practice to support patient outcomes.
• Multidisciplinary Working: Collaborate closely with doctors, nurses, physiotherapists, social workers, and other professionals to coordinate care.
• Patient and Carer Support: Provide advice, education, and support to patients, families, and carers to promote independence and wellbeing.
• Risk Management: Ensure compliance with clinical governance, risk management, health and safety standards, and professional guidelines.
• Supervision and Mentoring: Support, supervise, and contribute to the development of junior staff, assistants, and students.
• Service Improvement: Participate in audits, quality improvement initiatives, and service development activities.
• Record Keeping: Maintain accurate and timely clinical documentation in accordance with HCPC and organisational standards.

Qualifications / Requirements

• Occupational Therapy Qualification: Degree in Occupational Therapy or equivalent recognised qualification.
• Registration: Current HCPC registration as an Occupational Therapist.
• Acute Experience: Previous experience within acute hospital settings, discharge planning, or physical health services.
• Assessment Skills: Strong experience undertaking complex assessments and rehabilitation planning.
• Clinical Knowledge: Sound understanding of occupational therapy practice within hospital discharge pathways.
• Communication Skills: Excellent verbal, written, and interpersonal communication abilities.
• Teamwork: Ability to work effectively within multidisciplinary teams and independently manage a busy caseload.
• Driving Requirement: Full UK driving licence is essential.
