Role Overview:
As a Band 6 Mental Health Practitioner within the Young Persons Assessments Team, you will be responsible for undertaking urgent and routine mental health assessments for children and young people. You will work collaboratively with multidisciplinary colleagues to ensure timely, effective, and person-centred care.

What you will do:

  • Conduct urgent and routine mental health assessments for young people presenting with moderate to severe mental health needs.

  • Develop and contribute to individualised care plans in collaboration with multidisciplinary teams.

  • Provide support and guidance to young people and their families, helping them understand and navigate mental health challenges.

  • Maintain accurate, timely, and high-quality clinical records in line with NHS standards and policies.

  • Work effectively within the team to ensure safe, responsive, and high-quality service delivery.
