What you will do

• Manage a community stroke and neurological rehabilitation caseload.
• Undertake comprehensive Occupational Therapy assessments for patients with complex physical and psychological conditions.
• Develop and deliver individualised treatment programmes.
• Provide specialist Occupational Therapy interventions within community settings.
• Support rehabilitation and discharge planning.
• Represent Occupational Therapy within multidisciplinary team meetings.
• Provide specialist clinical advice regarding patient management.
• Complete outcome measures and monitor intervention effectiveness.
• Assess and prescribe equipment to support rehabilitation and independence.
• Maintain accurate clinical records, reports and correspondence.
• Supervise Band 4 Therapy Assistants and support students where required.
• Participate in audit, research and service improvement projects.

Requirements

• Degree or Diploma in Occupational Therapy.
• HCPC registration.
• At least 18 months to 2 years of post-graduate experience.
• NHS and community occupational therapy experience, with a focus on stroke rehabilitation and/or neurology.
• Experience within multidisciplinary teams.
• Experience supervising students or assistants.
• Strong clinical reasoning skills.
• Valid UK driving licence and ability to travel between locations independently.
• Car owner/driver.
