What you will do:

- Conduct initial and comprehensive assessments of health and social care needs, either individually or collaboratively, and communicate findings to the Multi-Disciplinary Team.
- Take on care co-ordination and co-worker responsibilities, serving as the Care Co-ordinator under the CPA process for an identified group of service users.
- Supervise a designated group of staff and students on placement within the team.
- Participate in Multi-Disciplinary Team activities, such as duty and clinic rotas, ensuring effective collaboration within your professional scope.
- Foster effective communication strategies between Mental Health Services and General Practice Teams to enhance patient care pathways.
- Provide ongoing contact with GP Practices as part of your link worker role, developing an understanding of the CMHT's role and function.
- Educate health professionals and other services on Secondary Mental Health service criteria and provide pre-referral advice and consultation for appropriate referral triage.
- Carry out Risk Assessments and formulate risk management plans according to Trust policies.
- Regularly attend clinical meetings to discuss patient/client needs, fostering a unified approach to care, treatment, and risk management.
- Ensure safe discharge processes to primary care when secondary services are no longer needed.
- Facilitate joint training events within the Organisation/Borough as appropriate.
