What you will do

• Serve as Care Coordinator for a caseload of adults with severe and enduring mental illness, ensuring care aligns with the Care Programme Approach (CPA) and recovery principles.
• Conduct comprehensive mental health, risk, safeguarding, and capacity assessments using Trust-approved frameworks.
• Develop and manage individualised care plans and relapse prevention strategies in collaboration with service users and carers.
• Deliver evidence-based therapeutic interventions while supporting individuals to achieve their recovery goals and maintain community independence.
• Take part in duty, crisis response, and urgent assessment functions within the Community Mental Health Team.

Person Specification

Qualifications

• Must be a Registered Mental Health Nurse (RMN).
• Hold current registration with the Nursing and Midwifery Council (NMC).
• Showcase evidence of continuing professional development relevant to community mental health practice.

Experience

• Possess significant post-registration experience within community mental health services.
• Proven experience supporting adults with severe mental illness, psychosis, complex emotional needs, and co-existing mental health conditions.
