Role overview:
As a Band 6 Community Physiotherapist, you will deliver high-quality rehabilitation services within the community, supporting patients following hospital discharge and contributing to intermediate care pathways. Your role will focus on promoting recovery, independence, and safe transitions from hospital to home.

What you will do:

• Conduct community rehabilitation visits following hospital discharge

• Support intermediate care discharge planning, ensuring safe and effective transitions

• Deliver rehabilitation programmes within the community to improve functional outcomes

• Work collaboratively with multidisciplinary teams to support holistic patient care

Person specification / requirements:

• Registered with the Health and Care Professions Council (HCPC)

• Minimum of 2 years’ physiotherapy experience, ideally within a community setting

• Car driver preferred to facilitate travel across community locations
