An exciting opportunity has arisen for an Adults Social Worker to join the Integrated Neighbourhood Team in Manchester.

What You Will Do

Manage a diverse caseload of adults with complex needs, including older adults, people with physical disabilities, learning disabilities, and mental health needs.

Complete Care Act assessments and support plans using a strengths-based, person-centred approach.

Lead and contribute to safeguarding enquiries and multi-agency safeguarding processes.

Undertake Mental Capacity Act assessments and Best Interests decisions, including involvement in DoLS or LPS processes where required.

Work closely with health colleagues to support admission avoidance, discharge planning, and community-based interventions.

Promote independence and wellbeing through the use of community resources and preventative services.

Coordinate complex care packages and commission services in line with council procedures.

Participate in multi-disciplinary meetings, case discussions, and care planning forums.

Maintain high-quality, timely, and accurate case recording in line with statutory and local requirements.
Requirements
Essential
-
Degree or equivalent qualification in Social Work.
-
Registered with Social Work England.
-
Completion of ASYE or equivalent post-qualifying experience.

Experience working in Adult Social Care.

Ability to manage complex cases, work autonomously, and operate effectively within a multidisciplinary environment.
