What you will do:
- Deliver high-quality nursing care to individuals with learning disabilities, autism, and complex needs in a community setting.
- Collaborate with a multidisciplinary team to develop comprehensive care plans tailored to each individual's needs.
- Conduct assessments and reviews of clients' conditions, ensuring care plans are up-to-date and accurately implemented.
- Provide education and support to clients, families, and carers regarding health promotion and disease prevention.
- Participate in team meetings, case conferences, and training sessions to maintain and enhance professional skills.
- Ensure all activities comply with relevant legislation, policies, and procedures while maintaining accurate records and reports.
