We are on the lookout for an innovative and skilled Band 6 Admiral Nurse who will be based within our dynamic Mental Health Nursing team in Guernsey, UK. What you will do:
- Provide specialist bio-psychosocial assessment and support for individuals with dementia and families with complex needs.
- Facilitate the education and training of staff in delivering evidence-based dementia care, supporting policy development, and quality improvements.
- Work collaboratively with other professionals to enhance the management of co-morbidities and frailty among individuals with dementia.
- Develop care plans for families, promoting positive risk management and understanding of changes in relationships due to dementia.
- Act as an advocate, ensuring the safeguarding of vulnerable adults and promoting individual rights and best interests.
- Participate in case conferences, and support the development and delivery of dementia training and education to staff.
- Support the evaluation of quality improvements and the development of policies and procedures for dementia care in line with best practices.
