What you will do

• Perform comprehensive assessments and devise tailored care plans for adults with complex speech, language, and swallowing issues across multiple care settings.
• Work independently while recognising and responding to changes in patient conditions, escalating situations when necessary.
• Provide leadership by guiding other staff in organising, implementing, and assessing specialised care.
• Encourage active patient and family involvement in care planning, respecting cultural and personal values.
• Promote and implement health education for patients and their support networks.
• Collaborate closely with multi-disciplinary teams to provide comprehensive care, ensuring effective communication and relationship building.
