What you will do
- Assessment: Complete Care Act assessments, reviews, and person-centred support plans.
- Case Management: Manage a varied caseload of older adults with functional and organic mental health conditions.
- Dementia Support: Work with individuals living with dementia and early-onset dementia to promote independence and wellbeing.
- Mental Capacity: Undertake Mental Capacity Act assessments and Best Interest decision-making.
- Safeguarding: Carry out safeguarding enquiries and contribute to adult protection procedures.
- Legislation: Apply the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and associated legislation in practice.
- Multi-Agency Working: Collaborate with Community Mental Health Teams, NHS professionals, GPs, carers, and voluntary organisations.
- Care Coordination: Support effective care planning and risk management through multidisciplinary interventions.
- Record Keeping: Maintain accurate case records and prepare high-quality assessments and reports.
- Professional Practice: Participate in supervision, multidisciplinary meetings, and continuous professional development activities.
- Strengths-Based Practice: Promote independence, resilience, and positive outcomes for service users.
Qualifications / Requirements
- Social Work Qualification: Degree in Social Work or equivalent recognised qualification.
- Registration: Current Social Work England registration.
- Experience: Previous experience within Adult Social Care.
- Legislative Knowledge: Strong understanding of the Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983, and safeguarding adults legislation.
- Communication Skills: Excellent assessment, interpersonal, and report-writing skills.
- Organisational Skills: Ability to manage competing priorities within a busy community team.
- Driving Licence: Full UK driving licence and access to a vehicle is desirable.
