What you will do:
- Conduct strengths-based assessments and create personalised support plans that cater to the needs of young adults.
- Collaborate with young individuals, helping them navigate their journey from adolescence to adulthood.
- Execute detailed assessments under the Care Act 2014 to ensure that all aspects of care provision meet legal standards.
- Perform Mental Capacity Assessments to evaluate and support individuals in decision-making processes.
- Engage with Education, Health and Care Plans (EHCPs), contributing to strategies under SEND legislation to support learning and development needs.

Requirements:
- A minimum of 3 years’ experience in adult social work, specifically within transitional age groups.
- Relevant social work qualifications and a thorough understanding of related legislation.
- Aptitude for conducting assessments and preparing comprehensive reports.
- A full employment history with explanations for any gaps.
