What You Will Do

• Carry out needs assessments for adults in the community to develop effective, personalised care plans.

• Work collaboratively with colleagues, external partners, and agencies to ensure holistic, high-quality support.

• Conduct timely reviews of care and support plans to ensure they remain appropriate and effective.

• Provide advocacy, guidance, and support to service users, promoting independence and wellbeing.

• Maintain clear, accurate, and compliant case records in line with legal and organisational standards.

• Engage in ongoing professional development, keeping up to date with legislation, policy changes, and best practice.
